Those who are referred for services are assessed for the immediacy of need and type of intervention appropriate to help alleviate the threat of homelessness. All services are offered based on the U.S. Department of Housing and Urban Development (HUD) Coordinated Entry model for prioritizing household needs.
Coordinated Entry uses a standardized assessment tool to assist and refer people experiencing housing insecurity to programs available through Tri-County CAP. Staff Housing Navigators connect people in Coordinated Entry with a network of community resources to enhance their capacity to successfully secure and maintain permanent housing.

211 NH is the connection for New Hampshire residents to the most up to date resources they need from specially trained Information and Referral Specialists. 211 NH is available 24 hours, 365 days a year. Multilingual assistance and TDD access is also available. For those outside of New Hampshire, call 1.866.444.4211.
